Firstly,the earliest age for cosmetic surgery is 18 for most procedures. Even with parental consent, such things are complicated, difficult, and not recommended.

I would suggest working on your confidence and possibly seeking counseling if your self-image leads to depression.
Working out should be your number one option. In many cases this can resolve minor issues such as the ones you mentioned. Cosmetic surgery should be considered only after this solution has been exhausted.
